What is Population Health Management?

Within this PHM framework, the EIM Solution supports healthcare providers, hospitals and healthcare systems in adopting the PHM Care Model by stratifying patients, employees, and underserved community residents into at-risk population groups to provide effective intervention and treatment programs for chronic disease.
