Referral of At-Risk Populations to Interventions

The adoption of Population Health Management (PHM) by healthcare stakeholders has opened the door to a significant opportunity for physical activity (PA) to be established as a standard in healthcare.  The consolidation of healthcare systems is accelerating the adoption of PHM, along with “meaningful use” compliance of electronic health record (EHR) systems, to ultimately improve population healthcare coordination.

Healthcare stakeholders are following the lead of medical providers (the health systems) who are adopting PHM for their patients, and also for their own employees and community outreach initiatives, with the ultimate goal of implementing interventions that will slow, stop, or reverse chronic disease progression.
