As the leaders of the community-care team, EIM Exercise Professionals engage referred clients in patient-centered care, targeted face-to-face intervention sessions and provide tailored content. These Community Resources must be trusted by Healthcare Stakeholders.
The specific physical activity (PA) intervention programs offered within a community include:
- Structured PA and health and dietary discussions
Lifestyle behavioral-change strategies
- Based on the latest evidence-based information provided by the American College of Sports Medicine (ACSM) and related health organizations.
The PA intervention programs are matched to each referred patient according to his or her chronic disease(s), risk level(s), physical and motivational readiness, and personal preferences.
EIM Places are qualified locations that are designated specifically to a community to meet safety standards, their level of convenience and other criteria necessary for hosting PA intervention programs. Each EIM Place is evaluated on a regular basis to maintain its EIM designation and ensure that all community resources can be trusted by the healthcare stakeholders.
